In order to be approved for a clinical rotation with CHRISTUS Santa Rosa Health System or CHRISTUS Children's, there must be a current affiliation agreement in place with your Clinical Program. You must also have already secured a preceptor. The Graduate Medical Education (GME) office does not assist in finding preceptors or coordinating your rotation schedules.
- Please note that if your preceptor is employed by Baylor College of Medicine (BCM), additional approval from BCM will be needed and that can take up to 6 weeks. The forms needed for BCM approval can be obtained from your preceptor or their designated representative. Once approval has been obtained, please provide a copy to our Medical Education department along with your Letter of Good Standing. You will not be allowed to onboard with CHRISTUS until BCM approval has been obtained. This will be required for each rotation department and/or preceptor.
To request a clinical rotation, please complete all of the forms below no later than 4 weeks before your anticipated start date:
Read the Confidentiality Agreement ahead of time and sign the agreement upon registration at the GME office.
Allied Health Student Orientation Requirements
Once you have reviewed all of the elements above, complete and submit the Student Orientation Completion Certificate.
You must bring a valid photo ID (driver's license or passport) to the GME office prior to your rotation start date in order to complete your registration process and obtain a badge. You will not be allowed access to our facilities without obtaining your CHRISTUS ID badge.
Returning allied health students are required to notify the Medical Education office of changes in rotation locations and dates at least two weeks before the rotation begins. Use this form to easily update this information for our records.
